No requests will be honored once a team has registered.* Team Check in will take place on Thursday and Friday evening from 6pm => 9pm at the Holiday Inn Westlake. 1100 CROCKER RD A coach or team representative must come to check-in night with the following completed items: 1. Two (2) copies of your official league team roster, signed by your state registrar 2. Two (2) copies of the guest player roster, if applicable 3. Medical release forms for each player (you may use your league’s medical forms or copies of the provided medical form) 4. All player and coaches passes (including guest player passes) 5. A travel permit, if applicable 6. The team Roster/Waiver Form 7. Team Contact Information Sheet All forms can be found at the link below or under the Forms tab above. *A single representative from a club may check-in multiple teams and pick-up coaches bags. However, each coach must have their approved, sealed roster (provided at check-in) in hand prior to the start of their first game.* **There will be no Saturday morning registration.
